Where are you located? Hours?
Endeveren Family Medicine is located at 3015 North 90th Street, Suite 1 in Omaha, Nebraska. Our green door faces the southwest corner of 90th and Maple Streets. Our office is open Monday through Thursday from 8:15am to 4pm and is closed Friday through Sunday and all federal holidays.
How can I reschedule or cancel my appointment?
We encourage patients to provide a minimum notice of 24-hours to reschedule or cancel their appointment, please give us a call at (402) 453-6869.
What insurance do you accept? Can I self pay?
We accept all major insurance companies and offer payment plans on a sliding scale to our valued patients who opt to self-pay. Please inform a team member of your insurance at the time of booking an appointment.
What are your COVID-19 policies?
To stop the spread of the COVID-19 (coronavirus pandemic), Endeveren Family Medicine requires all patients above the age of 5-years-old to wear a mask that completely covers their nose and mouth upon entering our office. Patients are limited to one (1) guest and encouraged to arrive no more than 10-minutes prior to their appointment time to avoid crowding in our waiting room.
